Henry Ford Innovation Institute (HFII) is seeking to attract entrepreneurs, engineers, developers, clinicians, researchers and other solutions providers with mobile-health and IT solutions that address the root causes of costly readmissions of patients with acute myocardial infarction, congestive heart failure, COPD, pneumonia, and total hip and knee replacement.

Objecte

Challenge is open to the following Contestants:

i. Individuals who are at least 18 years of age at the time of entry; and are legal residents of the United States or of a country where such challenges are allowed by the laws of such country and the United States.

ii. Teams of eligible individuals where each team member meets the eligibility requirements for individuals.

iii. Corporations (including nonprofit organizations), limited liability companies, partnerships, and other legal entities that, at the time of entry are legally formed, and employ fewer than fifty (50) people and have raised less than $10,000,000 in funding from any source; and are composed of individuals meeting the requirements detailed above.
